Shelly Evans:

Senior Director Post-Acute Consultant

With over 25 years of experience in the healthcare field, Shelly has worked in many capacities over her long career including; Business Development, Home Services, Post-Acute Services and Pharmacy. She has developed many post-acute departments and preferred community provider networks for hospitals and health care systems. Our senior consultant has lead health care systems, BPCI-A and Next Gen ACO groups in an effort to lower readmission rates, reduce patient length of stay, manage cost savings and overall Medicare spend, while significantly improving patient care, quality and safety. She uses her knowledge and expertise to drive specific health system Value Based Purchasing initiatives throughout the health care system and care continuum. She uses this foundation to develop and enhance post-acute relationships in the acute, LTACH, IRF and SNF settings and helps long term care facilities develop relationships with community hospitals. With this experience and remarkable outcomes in both patient care and financial savings, she can lead;

Jaime Redman:

Executive Clinical Consultant

Jaime has over 15 years of experience in the healthcare field. She has been a Nurse Practitioner in the Post-Acute setting for 6 years, while leading a Next Gen ACO Post-Acute care group of NPs in multiple SNFs within the community and enhancing BPCI program initiatives. Our clinical consultant has implemented multiple Post-Acute care strategies and assisted in building strong and productive relationships within preferred provider networks. She has educated providers that are giving care in this complex area on how to drive down length of stay, lower readmission rates and overall spend while improving patient care outcomes. By working as part of an interdisciplinary team with key members of partner facilities and other community services, our clinical consultant has identified techniques to assure alignment of goals for health systems, SNFs and ancillary businesses. With her assistance you will be able to implement;
